			<content:encoded><![CDATA[<p>As Americans we are constantly bombarded with the newest diet fad, diet plan and can find a slew of diet &#8220;tips&#8221; online. How do we know what works and what does not? In the 1980&#8242;s the craze was to go to the &#8220;meetings&#8221; where all the people sat around and talked about food. Then the &#8220;experts&#8221; (formerly students themselves on the plan) weighed you in front of everyone and you recorded it all in your journal that you toted around with you everywhere you went. No one talked about metabolic rate or fat to burn ratios because that simply wasn&#8217;t in &#8220;the plan&#8221;. </p>
<p>Then came the cards. Depending on the fad diet you were following you might have had colored cards (eat 5 green item cards and 2 blue) today or you were moving cards from the right side of the book to the left side of the book and when you ran out of cards you simply stopped eating for the day.     </p>
<p>In the 1990&#8242;s the fad was the weight loss clinic. Every &#8220;counselor&#8221; was more like a sales person that was concerned on how long it would take you to lose weight and based your cost on time not real progress. So for 100 pounds of weight loss it would take you 1 year and $2300. Then you got a book of approved foods and were told to keep a journal that would come in with you once a week for 52 weeks where you would be weighed and your journal evaluated. Sounds a lot like the &#8220;meeting&#8221; only the person behind the desk wore scrubs and seemed a bit more official.   </p>
<p>For the 2010 era, the newest craze is to use a nutritional tracker that can monitor your progress, offer tips and suggestions, and personalized nutritional plans according to the foods you like, coaching, even a fitness buddy, and all with the ease of being able to access it from your phone or computer. No group meetings, no need to trek anywhere to any clinics. No colored cards, no embarrassing weigh-ins. You are safe in your own home (or via phone wherever you go) and for a small fraction of the costs associated with the precious weight loss fads. </p>
<p>The following are some Team Fitness America reasons that nutritional tracking works quicker and more efficiently rather than using any of the methods listed above: </p>
<p>Back then:  You had to trek to the nearest &#8220;weight loss clinic&#8221; and meet with a young sales person that called themselves a weight loss counselor. They handed you a mini book of &#8220;approved foods&#8221;&#8216; and stuck you on a scale. Now, you have privacy and choices with many more options. </p>
<p>Online nutritional programs: Weigh yourself in the comfort of your own home with no prying eyes. No group weigh-ins and no feeling belittled in front of a group of people. Choose your own foods, plug them into the nutritional tracker and it will give you menus, tips, and ideas while allowing you to track your calories with proper fat intake right before your eyes. Some of the programs even offer videos of food preparation so you can see how exactly to make a healthy meal. </p>
<p>Back then: You had cards that you moved from the left side of the portfolio to the right side and when all your cards were gone, you were done eating for the day. But what if you were still hungry? Tough!     </p>
<p>Online nutritional programs: Choices for food and how much based on your exercise outputs, weight, goals and body make up. Things that are different among us all are now taken into consideration. Now we know that not every person loses or gains weight at the same rate as the next person. We also know that each person may metabolize certain foods differently simply based on chemistry, past medical history and health conditions. With cards or mini books there is no way to discern any one person from the next person. With a customized, personalized online system you can see how different your food choices are from your friends and you both will benefit.       </p>
<p>Back then: A hardcopy journal that you better not lose or your &#8220;counselor&#8221; will not be happy. </p>
<p>Online Nutritional tracking: Now it is all electronic so you won&#8217;t lose the journal and you can make entries from any place discreetly. The dinner table, a restaurant, the office, the subway, or any place you choose and no one will know that you are counting calories like if you broke out the journal and a pen. Plus all of your information is stored electronically for ease of access and accountability. You can have a coach look at your information the same time you are to make suggestions, keep you motivated and provide all the educational tools you would need to understand how your metabolic rate affects your weight. </p>
<p>Back then: A monthly fee plus start up costs, all based on how much weight you plan to lose. This can be in the thousands depending on how long you plan to use the program. </p>
<p>Online Nutritional tracking: One set fee, or a small monthly fee depending on your level of service. So it is the same if you lose 10 or 100 pounds and it is very affordable even for a small budget. </p>
<p>The advent of the online nutritional tracker may just put the weight loss clinic out of business with more to offer, ease of access and comfort of access from your own home or any location. Long gone are the days of writing down your food intake calories in a journal and handing them to your personal trainer to review and make suggestions on what to add or delete form your daily food intake. Now you can plug it all into a computerized system, make changes as you need to and have a live representative review it on the spot. </p>
<p>When you couple the online nutritional tracking with a solid personal training program, like one at Team Fitness America, you will achieve maximum results in a fraction of the time it would take with a food program alone.</p>

<p>Osteoporosis is an important factor in consequences and disease states. It can occur due to the lack of calcium intake, weight bearing activities, and estrogen deficiency. This disease affects over 25 million American women and causes about 250,000 hip fractures a year. It is called a silent disease, which causes bone density to slowly decrease and fractures to develop (Alexander, 2004 p. 267). After age 30, bone resorption speeds up faster than bone replacement can take place. When a woman reaches menopause she will experience approximately 20% of bone loss within a five to seven-year period. After age 50, one in two women will experience osteoporosis related fractures, which may lead to disability, chronic pain, and even death (Alexander, 2004 p. 399). The economic cost is overwhelming and significant for osteoporosis. This is why it is important to properly select the main focus point of long-term prevention or acute care of osteoporosis. It has been a complex decision, which has been a growing issue for women worldwide. Some issues that each program must over come in order to be an affective long-term prevention or acute care program are services reaching proper target audiences, maintaining ethics, effective budget spending, eligible health care services, increasing insurance coverage and personal knowledge.</p>
<p>In long-term prevention, preventive education of osteoporosis was a strategic tool to overcome this disease. Promotional education has been aimed at the premenopausal phase of women&#8217;s lifecycle. This phase represents promoting nutritional facts to help build stronger bones, which are aimed at children and adolescents as a general target audience. This is based on research of early lifestyle changes that promotes optimal adult peak bone mass. The specific target audience is low income and high-risk applicants who met categorical, residential, income, and nutritional risk requirements.</p>
<p>There are several Federal agencies as well as non-profitable organizations, which maintain ethics by providing strict guidelines. This allows participants to experience uncompromised services such as health education, specialized services, and sometime financial support (Food and Nutrition Service United States Department of Agriculture [FNS-USDA] n.d.). Some examples of these agencies are Women Infant Children (WIC), Expanded Food and Nutrition Education Program (EFNEP), U.S Department of Health and Human Services (DHHS), and National Dairy Council (NDC).</p>
<p>Another issue of concern is individual effective budget spending, which is measured through cost effective services. There are multitudes of diverse governmental and non-profit agencies that are operating jointly or individually in support of healthy lifestyles and stronger bones. The combination of overall promotional efforts is justified by a 50% decreased risk of osteoporosis, according to the Milk Pilot Study by the National Dairy Council (NDC, n.d.).</p>
<p>  Other issues such as eligible health care services and increasing insurance coverage are intertwining problems. In 2001, 41.2 million U.S. citizens were uninsured. There were a million more that had limited insurance coverage, which were restrictive and prevented necessary health care services (Alexander, 2004 p. 18). There are a growing number of 44 million uninsured citizens, which is partially due to the increasing elderly population (Connolly, 2000).  Affective long-term educational prevention should help decrease the massive amount of patients who are at need of medical assistance. This is due to reinforcing positive lifestyle changes to help prevent chronic illness. Defining and reducing additional risk factors to provide better prevention techniques such as avoiding high protein intake to avoid urinary calcium loss. Another prevention technique to reduce a risk factor is avoiding high fat intake to avoid reduced bone mineralization due to elevated needs of bisphosphonates for metabolic intermediates of cholesterol synthesis (Hegsted, 2001).</p>
<p>Personal knowledge of providing educational support allows us to overcome learning barriers that must be faced such as cultural differences and body image. Cultural background influences the ways adolescents think about things and interact with others.   It is important to consider the cultural aspects in order to break down social barriers and provide adequate behavior modification tools.  Especially when reviewing the many factors that contribute to an adolescent&#8217;s culture such as socioeconomic, educational, family, ethnic, and racial background (Conflict Research Consortium, n.d.). The other barrier is body image, where proper support of nutritional education and counseling becomes necessary for adolescents&#8217; development. This becomes evident as self-perception traits become defined in the preteens. If they carry their negative association of dairy products into adulthood, their risk of osteoporosis will become great. Surveys indicate dairy products are considered to be calorie dense food items (Adolescent Health Committee &amp; Canadian Pediatric Society, 2004). Negative self-perception contributes to low calcium intake, which in turn contributes to a decrease in adult peak bone mass (DHHS, n.d. &amp; Teegarden, 1999).</p>
<p> I have learned in this class that today&#8217;s adolescent development could lead to tomorrows growing issues of chronic problematic illness such as osteoporosis. I question the overall effectiveness of long-term educational prevention of osteoporosis. According to the National Osteoporosis Foundation, more than 25 million Americans have osteoporosis (DHHS, n.d.). 50% of patients with hip fractures will be considering long-term disabled. 25% of those patients will require long term-nursing facilities (The Bone and Joint Decade [BJD], 2004).  If National Osteoporosis Foundation was able to receive stronger Federal support to coordinate state agencies in joint, local, as well as national effort, the overall program will become effective. This is why it is important to properly select the main focus point towards long-term prevention.</p>
<p>Addressing the issues of effective osteoporosis therapy in an acute care setting is an ongoing process.  This type of health care service is always changing towards more efficient techniques. Currently doctors recommend healthy lifestyle of nutritional dense foods and weight bearing activities. They prescribe medication for bone resorption such as Fosamax and Calcitonin (Alexander, 2004 p. 403). They also prescribe hormone therapy such as Raloxifene, which reduces 50%-70% vertebral fractures (Alexander, 2004 p. 402).</p>
<p>Services reaching proper target audiences in the past were questionable. According to multinational study of orthopedic surgeons, 95% fracture patients were not properly screen for osteoporosis (BJD, 2004).   Another target audience that has been over looked were patients who were experiencing celiac disease. It is common for these individuals to be lactose intolerant as well, which puts them at risk of osteoporosis (American Academy of Family Physicians [AAFP], 2005). Screening is an important tool to reach, as well as confirm, proper target audience.  Currently, selecting patients for screening has drastically improved, making it possible to catch osteoporosis in the beginning stages. Individuals who are experiencing estrogen deficiency, vertebral abnormalities, long-term glucocorticoid therapy, or hyperparathyroidism can be properly screened (Albert Einstein Healthcare Network [AEHN], n.d.).</p>
<p>When maintaining ethics, there should be a clear path of treatment for patients.In the pastOrthopedists were not consistent in treatment or referrals. It was an experimental practice, which was vastly growing through trial and error. Medical decisions were made by doctors without long-term clinical research (BJD, 2004). Currently patients are being offered choices of preferred treatments. The diverse pharmacologic prevention consists of teriparatide, raloxifene, bisphosphonates, and salmon calcitonin. Bisphosphonates were recommended as the most effective pharmacologic prevention for osteoporosis (Wright, 2004). </p>
<p>Effective budget spending of related osteoporosis costs were from admirable. In 1995, the cost of treating osteoporosis acute and long-term care was 13.8 billion American dollars. Currently more high-risk patients are experiencing early detection and treatment through advanced screening and medical attention. This will have a plausible influence on budget spending. The increasing number of geriatric needs will always be a growing challenge for effective budget spending (DHHS, n.d.). Eligible health care services and increasing insurance coverage were a challenge for people when faced with chronic diseases such as osteoporosis. Individuals often struggle to pay for appropriate treatment for long-term illness. Most people were forced to turn to Medicaid as a public insurance program (Alexander, 2004 p. 398). Currently conditions have been improving for patients with osteoporosis. According to National Osteoporosis Foundation (NOF), Medicare will reimburse bone density tests such as DEXA scans. Due to the direct reimbursement of Medicare, insurance will be able to cover these types of testing, which allows for early detection of osteoporosis (AEHN, n.d.).</p>
<p>Personal knowledge of providing clinical research allows us to overcome barriers such as cultural differences and body image. Cultural differences could develop into issues for osteoporosis patients such as 54-year-old raw food vegetarians who are at high risk.  This is due to alternative dietary lifestyles of high fiber, low vitamin D and calcium intake.  Clinical research indicates lactovegetarians have decreased risk factors compared to raw food vegetarians. A transition from raw food vegetarianism to lactovegetarianism is smooth and advisable for individuals (Hitti, 2005). Another barrier is body image. In 1998 the Gallup poll found 59% of women ranging 35-49 years of age were on some kind of diet.  These diets are known as yo-yo diets, which are hazardous to your health. This is due to overall decreased food intake, which affects macronutrients as well as micronutrients.  This includes important needed items of the bone matrix such as calcium, phosphorus, magnesium, Vitamin K, and Vitamin D.  Another growing concern dealing with body image is individuals who are currently seeking weight loss through Bariatric surgery. This procedure pertains to a partial bypass of the duodenum, which is an important part of the small intestines. This causes a deficiency of calcium due to chronic malabsorptionand puts the patient at risk for osteoporosis. Clinical research indicates progressive atrophy of the unused duodenum, which stresses the importance of corrective surgery to prevent significant chronic damage (Strong Health Bariatric Surgery [SHBS], n.d.).   </p>
<p>I have learned in this class there could never be enough research in order to make a precise decision. This is why additional financial aid through governmental and non-profit agencies is greatly needed for proper clinical research. Completing additional research will make acute care for osteoporosis more affective.</p>
<p>In conclusion, acute osteoporosis therapy is dependent on long-term prevention success. This is due to promoting awareness and managing risk factors that are modifiable. These techniques have been proven to decrease a large demand on acute care.   This directly causes a decrease in potential target audience for acute care. Maintaining effective budget, providing eligible health care services, and available insurance coverage is easier when there are less demands due to decreased needs of medical services. Both programs are necessary in order to provide optimum health care of women nationally. </p>

			<content:encoded><![CDATA[<p>Optimal nutrition is vital for proper organ function, wound healing, reducing the catabolic state following injury or stress, and for maintaining the immunity and general health of an individual. When a person loses the ability to eat or when a normal diet fails to meet one&#8217;s nutritional needs, it is certainly a cause for concern.</p>
<p> </p>
<p>Gastrointestinal diseases often lead to discomfort while eating, also causing malabsorption and anorexia. Diseases like sepsis, trauma, and cancer may induce muscle wasting and weight loss due to altered nutrient utilization. Also, in conditions associated with critical illness or surgical mechanical trauma, delay in feeding can postpone recovery and further lead to bacterial translocation from the intestinal cells into the lymphatic circulation, causing complications like sepsis and multiple organ dysfunction syndrome (MODS).</p>
<p> </p>
<p>Specialized nutritional support needs to be administered to maintain the nutritional status of the patient and to promote recovery. The most common specialized therapeutic nutritional systems are enteral and parenteral.</p>
<p> </p>
<p>Enteral nutrition (commonly known as tube feeding) is suggested for patients whose gastrointestinal tract is functional and who have diminished oral nutrient intake. The nutrients are delivered directly to the gut by the use of naso-gastric, naso-duodenal or naso-jejunal tubes.</p>
<p> </p>
<p>The feed can be given as a bolus (at one go), continuously, cyclically, or intermittently based on the patient&#8217;s requirement. The enteral feeds that are commonly used are homemade blenderised foods (low cost, especially used in developing countries) and other commercial feeds, which may be polymeric or elemental.</p>
<p> </p>
<p>On the other hand, parenteral nutrition is used to feed a person intravenously (directly through the blood stream). It provides nutrients to the patients whose gastrointestinal tract is nonfunctional due to either interference in its continuity or because of its impaired absorptive capacity. Predigested elemental formulas containing salts, glucose, amino acids, lipids, and added vitamins and minerals are administered. The gastrointestinal tract is completely bypassed.</p>
<p> </p>
<p>In some cases, parenteral feeding can also be combined with enteral feeding to meet the food requirements. When there is no food given by other routes it is termed as total parenteral nutrition (TPN). However, if these feeds are given for a very long duration, provided inappropriately, or without proper precautions, they may lead to severe adverse side-effects.</p>

The reason for this is because of the nutritional quality of the food that is made available. For millions of years our ancestors lived off the land and it wasn&#8217;t really until the industrial revolution that things started to go pear shaped. Large numbers of people flocked to the cities to fuel the need for labourers. To feed these workers, foods were refined (or processed) as a cheap form of energy. Processing foods also meant that they lasted longer.</p>

<p>The resulting health issues were serious. At the time there was very little understanding of food nutrition and how to supply the body with everything it needs to function properly. It was thought that food was just energy, just like any mechanical engine needs fuel. There was no understanding of micronutrients such as vitamins, minerals, etc. They had limited knowledge of the complex nutritional needs of the human body and therefore health declined rapidly.</p>
<p>Defining the problem</p>
<p>Over the last few decades in particular we have become increasingly health conscious. The only problem is that a lot of what we are taught about nutrition is blatantly not true. With the discovery of micronutrients health has not got better, instead it has got worse. When vitamins and minerals were first discovered it was thought that they were the cure for the wide-spread health problems. The solution seemed simple, just add vitamins and minerals to our food.</p>
<p>Has it worked?<br />Since vitamins and minerals were discovered cancer rates have increase 75 years in a row! Obesity and diabetes have also increased dramatically.</p>
<p> </p>
<p><strong>Defining optimum nutrition and the 2 keys to good health</strong></p>
<p>Optimum nutrition is simply where you give your body enough nutrients so that it is able to function at an optimal level.</p>
<p>The benefits of optimum nutrition include:</p>
<p>- Increased physical and mental performance<br />- Better moods<br />- Better sleep<br />- A longer, healthier life span<br />- etc&#8230;</p>
<p>The thing to understand is that everything that we do nutritionally nowadays is completely different to what it used to be like and this is a big problem. Even cooking food should not be a regular thing in your diet. Fire is a destructive force on nutrients and creates harmful substances. For millions of years before fire was discovered, everything was eaten raw.</p>
<p><strong> </strong></p>
<p><strong>1. Eat the majority of your foods raw</strong></p>
<p>This is something that I have covered briefly in a previous post. All foods are at their most nutritious when eaten raw. The majority of your diet should be raw fruit and mainly vegetables. The nutritional quality of green leafy vegetables such as spinach, kale, etc is far higher than any other food.</p>

<p><strong>2. Chew your foods well</strong></p>
<p>When you eat your foods raw you need to chew them well. This is because chewing is the first step in the process of digestion. If you don&#8217;t chew your food well then a lot of nutrients will pass through undigested.</p>
<p>The main point here is that we are designed to eat certain types of food. Unfortunately a lot of these foods are replaced with alternatives that the body struggles to deal with. A lot of the symptoms that most people think are normal are strong indicators that something needs to change.</p>

Our body I made up of chemical component such as water, proteins, lipids, DNA, and carbohrdrates. We neeed to take all the supllement in a proper and balance manner. When nutrition supplement is take in amounts greater than the recommmanded dietary allowance they will no longer will be nutrition and they will be considred as durgs. </p>

Many people eat anything and anytime which is unknowly or knowly effect your health and diet which will result in health problems, over wieight and lazyness. Nutrition products are alawys fresh and well sealed and always make you feel better. </p>

Currently, too many nutritional benefits are processed, cooked or fried out of what we eat, which leaves us little choice but to supplement our diet with nutrients such as vitamins and minerals to regain health and to stay healthy. If you add to this the pollution and other toxins you have to put up with, nutritional supplements become even more of a necessity</p>

Once you give your body a hand by giving it proper fuel, such as a diet high in fruits and vegetables, along with a sensible set of nutritional supplements, you will soon notice a difference. Healthy food and good nutritional supplements will restore your engine to optimum working order. It will thrive and so will you. DO NOT self-medicate with large doses of vitamins, minerals, or other nutritional supplements without seeking medical advice. </p>
